"This toolkit, for healthcare providers, assists in improving patient-centeredness during discharge through the delivery of a Patient Oriented Discharge Summary (PODS). Within each step of the process are tools in facilitating that step of the initiative: • Understand a PODS initiative • Assess your current discharge process • Implement a PODS initiative Within the implementation process, the tools provided are used to train and educate staff, patients, and caregivers about what PODS is, how it was developed and tips for success.
