Engaging Patients in Communication at Transitions of Care

Commissioned by the Australian Commission on Safety and Quality in Health Care, this report examines current evidence on the engagement of patients in communication at transitions of acute care and details effective tools, strategies, and resources to facilitate this engagement. The report found five key themes that enable engagement at transitions of care:
• Organisational commitment to patient engagement;
• Organisational culture and norms;
• Individual health care provider’s orientation and actions;
• Understanding and negotiating patient preferences; and
• Enacting information sharing and communication strategies.
The strategies and resources included in the report facilitate development and improvement in these areas.
