This tool is a short checklist of questions designed to be used by staff and physicians to explore their attitudes toward patient and family involvement. It asks questions about both patient and family involvement in individual care, as well as in advisory roles and/or members of improvement and redesign teams. It can be used for self-reflection and as a way to spark discussion among staff and physicians before beginning to work with patients and families as members of advisory councils, and quality improvement, policy and program development, and health care redesign teams.
