Learn more about Enabling Aging in Place, a new Healthcare Excellence Canada (HEC) collaborative focused on supporting the implementation of promising practices that enable older adults with health and social needs to age where they call home with improved safety, health and quality of life.

The collaborative will bring together health and social service organizations from across the healthcare continuum, supporting them to create an implementation and evaluation plan that helps them develop a person-centred and sustainable aging in place program.  HEC supports collaborative participants by providing seed funding, access to evidence and resources, evaluation support and learning, coaching and networking opportunities.
