Rethinking Patient Safety: Navigating the Measurement and Monitoring Safety Clouds

Leaders are key champions for Rethinking Patient Safety. This activity encourages self-reflection and discussion by leaders in five domains of the Measurement and Monitoring of Safety Framework (MMSF), which has formed the foundation for Rethinking Patient Safety. It is intended to support reflection and discussion around current approaches for measuring and monitoring safety in all sectors of healthcare – from acute care and rehabilitation, long term care, and care in home and community. While we have made progress on reporting and learning from harm, it is now time to maximize learning from other sources of safety information. Each dimension of MMSF is represented as a ‘safety cloud’ composed of two themes and questions to reflect on and discuss. 

Who this activity is for

This activity is for leaders and board members in healthcare organizations. It is designed to help leaders self-reflect and better understand how to support creating environments for safety, ensure improvement efforts are part of learning from harm, and that everyone has an opportunity to contribute to safer care.

Why it matters

Rethinking Patient Safety encourages us to recognize that everyone contributes to patient safety. Participating in this activity is an important starting point to creating safer care. It also provides an opportunity for your own continuing development. You can use the participant workbook in accreditation and interviews with regulators to demonstrate how you, as a leader, are learning.
