Last year, Healthcare Excellence Canada’s Reimagining LTC program supported over 230 long-term care (LTC) homes across 10 provinces to work together to build better care with and for people living and working in LTC in Canada. The latest issue from Long Term Care Today, Ontario Long Term Care Home Association (OLTCA)’s magazine, highlights how the program helped participants address gaps in the safety and quality of care in LTC, by providing more support to healthcare workers and building capacity to make care more person-centred.
Participating LTC homes covered a variety of key topics related to person-centred care improvement including:
- Innovative approaches to understanding lived experiences of residents
- Individualized care planning
- Activities to engage residents
- Improved dementia care quality
By the end of the program, teams working on person-centred care projects saw positive impacts related to staff and resident interactions – from how staff handle responsive behaviours to the involvement of family and essential care partners.

- Attendance and turnover
- Satisfaction

- Knowledge and confidence
