The Champlain BASE™ eConsult Service
The Champlain BASE™ (Building Access to Specialists through eConsultation) eConsult service is a secure online platform connecting primary care providers and specialists. In many cases, it prevents the need for specialist referral of patients and enables the primary care provider to take timely action on next steps of patient care.
The service offers patients more efficient, integrated and coordinated care, and provides improved access to specialty care using low-cost, easy-to-use technology.
Primary care providers and specialists developed the service to alleviate negative impacts on patients’ mental and physical health resulting from increased wait times to access specialist care. The service supports ongoing quality improvement through continuous evaluation and data analysis.
This innovation was spread as part of the Connected Medicine collaborative.
